Order Form

Minimum $10.00 per order.

This order form is a guide only.  If you prefer, I will accept any phone, email or written orders. Payment can be made by cheque, money order, Australia Post C.O.D., or Credit Card via PayPal.

Step 1:  Type in your personal particulars. If sending multiple order forms, you need only fill this section out on one form.

Step 2:  Select the d.p.i. resolution required for each batch of film or print scans. Please use only one dpi setting per order form. Use as many forms as you like. (If unsure see scanning resolution).

Cost per image ($Australian)

Step 3:

Select Print/Negative/Transparency numbers to be scanned: (1st number to appear under image on film strip or number marked on rear of prints) 

Step 4:

Select Print/Negative/Transparency numbers to be printed/enlarged: (1st number to appear under frame on film strip or number marked on rear of prints) 

Please ensure numbers are marked clearly on rear of photographs or prints.

Step 5:  Add any special Instructions.

Step 6:  Fill out this form and calculate the total cost.

Step 7:  Print this form out and check for any errors or omissions.  Post form with your slides/negs/photos to: Image Revival, PO Box 38, Dernancourt, SA, 5075.  I will email you with a full quote or credit card payment link.  Only Australian currency will be accepted unless using PayPal.  Services and products will not be posted until payment has cleared.  Confused? e-mail enquiries@imagerevival or phone me on (08)83961425 or 0412060385.

If you are unsure of the costs involved or are wary about sending payment prior to delivery of goods, please use Australia Post C.O.D. (Cash on delivery).  You can then pay for the goods and postage on delivery.
